| 1. How do I apply for a medical report?
There are a few ways you can apply for a medical report. They are:
a. Complete the clinical abstract application form and send it back to us together with a cheque made payable to 'National Heart Centre of Singapore Pte Ltd'.
b. Visit us at the Medical Records Office, Medical Reports Section, National Heart Centre Singapore. The operation hours are as follows:
Mondays - Fridays 8.00am - 5.00pm Saturdays 8.00am - 12.00pm
Closed on Sundays and Public Holidays
2. What do I need to bring when collecting the report?
You need to bring along your NRIC when collecting your medical report.
If you are collecting the report on someone else's behalf, you need to produce your NRIC and get an authorisation letter from the patient.
If the patient is deceased, please bring along a copy of the death certificate.
3. How do I make the payment?
Payment can be made using cash, credit card, cheque or NETS. Cheques should be crossed and made payable to ' National Heart Centre of Singapore Pte Ltd'.

Please quote the name and NRIC of the patient on the back of the cheque. Please do not send in any cash by post. You may pay in cash, NETS or credit card at the Medical Records Office.

Fees
- Completion of Simple Insurance Form by records staff $21.40
- Ordinary Medical Report / Complete Insurance Form $86.65
- Specialist Medical Report $173.30
